You will be asked where you lost your item, details about the item and your contact information.  If you are unsure about where you lost your item, you can select multiple locations.

Personal information submitted in this form is collected under the authority of the Municipal Act, 2001 2001, S.O. 2001, c.25 and in accordance with the Municipal Freedom of Information and Protection of Privacy Act, R.S.O. 1990, c.M.56. and will be used to contact you if your lost item is located.
